Precise Athletics and Self Defense (including all remote locations for online training), I the student/participant/parent/guardian acknowledge the existence of certain inherent risks in this type of training and hereby agree to assume all risk and responsibilities. I further relieve Precise Athletics and Self Defense LLC., its management, assigned staff, guest instructors and fellow students/clients from any liability resulting from personal injury or loss of or damage to personal belongings. I also hereby state that the student/client named above is physically fit to take the prescribed course of instruction and do so of their own free will for an agreed upon fee.
